GDPR: Consent Clauses and Information Clauses in the Processing of Personal Data
1. Information Clause: Processing for the Purpose of Providing or Selling Educational Services
In accordance with Article 13(1) and (2) of Regulation (EU) 2016/679 of the European Parliament and of the Council (General Data Protection Regulation) of 27 April 2016, I inform you that:
1) the controller of your personal data is eTrapez Educational Services E-learning Krystian Karczyński, ul. Piaskowa 101 lok. 4, 72-010 Police,
2) your personal data will be processed for the purpose of fulfilling the contract for the sale of educational services, i.e., Courses and Forum subscription, based on Article 6(1)(a) and (b) of the General Data Protection Regulation,
3) your personal data will be processed for the duration of the contract, and after its completion, they will be stored for the period of limitation of public law claims, i.e., 6 years,
4) you have the right to request access to your personal data from the controller, their rectification, deletion, data transfer, or restriction of processing, as well as the right to withdraw consent to the processing to the extent that the processing was based on your consent, and the right to object to the processing,
5) the right of access to personal data can be exercised by submitting a request for access using the form available on the Administrator’s website,
6) access to the personal data of the data subject will be granted after positively verifying their identity, for the security of their personal data,
7) you have the right to lodge a complaint with the supervisory authority,
8) providing personal data is voluntary, however, refusal to provide data may result in refusal to conclude a contract
2. Information Clause: Processing Personal Data for Own Marketing Database
In accordance with Article 13(1) and (2) of Regulation (EU) 2016/679 of the European Parliament and of the Council (General Data Protection Regulation) of 27 April 2016, I inform you that:
1) the controller of your personal data is eTrapez Educational Services E-learning Krystian Karczyński, ul. Piaskowa 101 lok. 4, 72-010 Police,
2) your personal data will be processed for the purpose of own marketing based on Article 6(1)(a) of Regulation (EU) 2016/679 of the European Parliament and of the Council (General Data Protection Regulation) of 27 April 2016,
3) your personal data will be stored until the request for removal from the subscription,
4) the recipients of your personal data will be entities cooperating with the Administrator, i.e., IT service and email hosting service providers, to the extent necessary to fulfill the contract concluded with the Administrator,
5) you have the right to request access to your personal data from the controller, their rectification, deletion, data transfer, or restriction of processing, as well as the right to withdraw consent to the processing to the extent that the processing was based on your consent, and the right to object to the processing,
6) the right of access to personal data can be exercised by submitting a request for access using the form available on the Administrator’s website,
